Video-assisted two-stage basilic vein transposition for creation of brachio-basilic arteriovenous fistulae

We report our experience in a mixed minimally-in- vasive technique for the two-stage transposition of basilic vein on a small series of eight patients. The operative tech- nique consisted of a modified endoscopic (1) approach for the two-stage transposition (2). The intervention was performed under Brachial Plexus Block (3).

Ever since 1879, when Josef Hyrtl first formulated his thesis that the names of the cephalic and basilic veins, as applied to the arm veins, were of Arab origin, a discussion began between philologists and historians of medicine as the former defended the Greek source of such denominations whilst the latter shared the view of the Viennese anatomist. The author, after making a critical review of the -relevant works published hitherto, unearthes a text drawn from a Persian manuscript dating back to the 15th century that, in his view, confirms the rigthness of the Viennese anatomist's thesis.

BACKGROUND Prosthetic infection after reconstructive vascular surgery is a most serious complication, associated with high mortality and amputation rates. Following excision of the infected graft, several methods of reconstruction are available. We present here our experience with autogenous reconstruction of infected prosthetic arterial grafts using the superficial femoral vein (SFV). METHODS From November 1995 to December 1999, we used the SFV in seven patients (mean age 70 years) for reconstruction of an infected aortobifemoral (n = 2), aortoiliac (n = 1), femorofemoral bypass (n = 1), femorotibial (n = 2) and carotid crossover bypass (n = 1).
